Step 2: Fill in and return the information forms

Since the 8th of February 2022, the evaluation of a support need is done as much as possible online. Read more about this via Evaluatie ondersteuningsnood | Kind en Gezin.

Create an account with Mijn Kind en Gezin, if you do not yet have an account. Please follow this step-by-step plan.
Fill in the psychosocial information form by clicking on the button labeled Specifieke ondersteuningsnood, which is to be found on Mijn Kind en Gezin. Fill in the form completely.

Tip: If necessary, ask someone who is very well acquainted with your child and your family and whom you trust for assistance.

Immediately schedule an appointment with your attending physician (the doctor who is in charge of your electronic medical file) to fill in the medical information form together. Your doctor can find this form in eHealth, under 'Evaluatie ondersteuningsnood Opgroeien'.

You have 4 months to fill in the two information forms.
